Royal Congregate Living Facility is a California Department of Public Health–licensed 24-hour congregate living health facility (CLHF) in Lancaster. We deliver skilled nursing, post hospitalization and subacute care to individuals with a wide range of conditions. Our patient conditions include the most complex cases of Traumatic Brain Injuries, Cerebrovascular Accidents, neurological diseases or following prolonged hospitalization or surgery. Alongside nurses, therapists, social workers, and other clinical support staff, our management team ensures your loved one receives attentive day-to-day care, coordinated therapy, and true “royal treatment” for the best possible recovery.

Rehabilitation & Therapy

We’re elevating recovery at Royal Congregate Living Facility with comprehensive subacute rehabilitation delivered by highly trained, experienced staff. Services include physical, occupational, and speech therapy. Our Rehabilitation team works to engage each patient with therapeutic activities that promote lasting independence, comfort, and reintegration.

Care Coordination

Royal Congregate Living Facility provides proactive care coordination to keep treatment seamless from admission through discharge. We liaise with physicians, specialists, hospitals, therapists, pharmacies, and payers to secure authorizations, arrange services, and clarify benefits. We assist in setting up all your appointments to physician offices, diagnostic tests, treatments such as chemotherapy and radiation for cancer patients to ensure continuity of care which is optimal for healing.

24-Hour Inpatient Care

Royal Congregate Living Facility provides true 24/7, around the clock inpatient care in a home-like setting. Our skilled nursing team coordinates with a 24 hour on-call physicians and Nurse practitioners to support complex recovery needs. The facility is staffed with a Licensed Vocational Nurse and a Certified Nursing Assistant 24 hours a day and 7 days a week. Registered Nurses oversee and supervise patient care and are readily available to provide guidance and support when needed. Hence we can support patients with complex needs including complex wound management, IV therapies, TPN and G-tube feeding.

Wound Care Management

Royal Congregate Living Facility delivers exceptional wound care management for complex, high-risk wounds. Our nurses perform skin assessments, measure and document progress, coordinate physician orders, and provide dressing changes, debridement coordination, pressure-injury prevention, offloading, and pain control. We educate families and monitor for infection to promote healing.
